Two die in Japan days after receiving Moderna shots suspended over contamination risk

Tokyo, Japan (CNN)Two people have died in Japan days after receiving doses from a batch of Moderna Covid-19 vaccines whose use was suspended Thursday following concerns over a contamination risk, the country's Ministry of Health, Labor and Welfare said Saturday.

A causal link between the vaccine and the deaths has not yet been established, according to the ministry and Takeda Pharmaceutical Company, which distributes the Moderna vaccine in Japan.

Japan suspended the use of about 1.63 million doses of the Moderna vaccine on August 26 as a precaution after foreign substances were found in some vials. 

While inquiries are being conducted by the Japanese government and three pharmaceutical companies involved, no police investigation has been announced and there is no present indication of wrongdoing. 

"Currently, no link has been confirmed between these deaths and the Moderna vaccine, but we believe it is important to carry out a formal probe to investigate the matter further," Takeda Pharmaceutical Company said in a statement released Saturday.

Japan's Ministry of Health will conduct that investigation alongside Takeda, the company stated, adding that Moderna has its own investigation underway. 

Rovi, a Spanish company that manufactured the batches, described the contaminant as "particulate matter" in a statement Thursday. Rovi said it is also investigating the contaminated lots.

The 1.63 million doses came from three identified lot numbers -- 3004667, 3004734 and 3004956. The doses had been distributed to 863 vaccination sites across Japan, and some were used. 

The health ministry said it began notifying the vaccination sites on Thursday morning about the contamination risk.

Japan's Moderna vaccine contamination woes widen as 1 million more shots suspended

TOKYO (Reuters) -Moderna Inc's COVID-19 vaccine contamination woes in Japan have widened with another million doses being temporarily suspended, after foreign substances were found in more batches and two people died following shots from affected lots.

The suspension of Moderna supplies, affecting more than 2.6 million does in total, comes as Japan battles its worst wave of COVID-19 yet, driven by the contagious Delta variant, with new daily infections exceeding 25,000 this month for the first time amid a slow vaccine rollout.

The latest reports of vaccine contamination came from Gunma prefecture near Tokyo and the southern prefecture of Okinawa, prompting the suspension on Sunday of two more lots in addition to the 1.63 million doses already pulled last week.

A tiny black substance was found in a Moderna vaccine vial in Gunma, an official from the prefecture said, while in Okinawa, black substances were spotted in syringes and a vial, and pink material was found in a different syringe.

Japan's health ministry said some of the incidents may have been due needles being incorrectly inserted into vials, breaking off bits of the rubber stopper. Other vials from the lots can continue to be used, the ministry said on Monday.

Keep in mind that nothing like this has been found in any of the other batches anywhere else in the world and the vaccines have been proven to have enormous life saving and severe illness saving benefits compared to their very small risks.

I added pertinent links as they related to the specific company responsible for the contaminated vaccine.  Details of both how and where, in what country and what kind of facility.  The issue of quality control is relevant, how are qualified personnel found and trained in odd places like Grenada? The subject heading of this original post concerns contamination of Moderna batches in Japan.  Every one of my "additions" highlight additional aspects of that specific topic, even referencing 'Fierce Pharma', an industry news portal.  I deliberately refrained from any personal interpretation.

Foreign substances in Japan's Moderna shots: Here's what you need to know

An investigation by Takeda showed that there’s a high possibility that the black foreign material was rubber that was shaved off when the syringes pierced through the rubber stoppers of the vials. As for the pink substance, that material is too big to be introduced by a syringe into a vial, so it may have already been inside, the ministry said.
